During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 10/07/2024 and completed on 10/16/2024. The on-site inspection began at 9:15 am and ended at 10:45 am. The inspector reviewed compliance in the areas listed above. There were 23 children present and 5 staff. The inspector reviewed 5 children?s records onsite and 5 staff records electronically on 10/16/2024.

This inspection included document review, tour of the facility, interviews, observations, and measurements.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance.

There was no physical examination in the record of Child #4, who had been enrolled for over 7 weeks.
Plan of Correction: Provider will obtain.
Standard 8VAC20-780-240-A
REPEAT VIOLATION
Staff are to complete the VDOE sponsored online orientation course within 90 days of hire.

Staff #5 completed the course three months late

Standard 8VAC20-780-330-B
REPEAT VIOLATION
Where playground equipment is provided, resilient surfacing shall comply with safety standards and shall be under equipment with moving parts or climbing apparatus to create a fall zone free of hazardous obstacles.

The resilient surfacing measured less than an inch at the end of the yellow and purple slides and in multiple places under the equipment. Grass was observed in multiple areas of the fall zones.
Plan of Correction: Provider will contact the groundskeeper/landscaper.
