Inspector Notes

An unannounced renewal inspection was conducted on December 6, 2022. There were 7 children with (18 points) with 3 caregivers. The home, children's records, caregiver records, menu's, first aid kits, emergency preparedness plans and procedures were reviewed. The children were observed singing songs and playing an alphabet game. The lunch to be served was chicken fingers, broccoli, cantaloupe. Areas of non-compliance were cited on the violation notice. Standard 8VAC20-800-100-A
Based on a review of children's records, the provider failed to obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.

Evidence:
Child #1 and #2's records do not contain documentation of a physical examination.
Plan of Correction: I will get the physical with the office stamp and signature from the parent and will place it in the child's file.
Standard 8VAC20-800-200-A
Based on a review of caregiver records, the provider failed to orient the assistant by the end of their first week of assuming job responsibilities.

Evidence:
Caregiver #2's record does not contain documentation of orientation.
Plan of Correction: I will complete the documentation of orientation and sign it and have the assistant sign it and then I will place it in her file.
Standard 8VAC20-800-60-A
Based on a review of children's records, the provider failed to obtain all required documentation for each child enrolled.

Evidence:
Child #2's record does not have documentation of one emergency contact.
Plan of Correction: I will get the information and place it in the child's file.
