Inspector Notes
Today an unannounced Renewal Inspection took place between the hours of 11:00 am and 2:15 pm. There were six (6) classrooms observed with a total of 59 children with 10 staff within the supervision guidelines. The children were observed playing with manipulatives, story time, diaper changing, lunch and other organized activities. A complete inspection of the physical plant, children and staff records, fire drill log, medication and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Violations
6Evidence:
1.Child #1 started on 9/9/22 and immunization is dated 9/30/22.
Evidence: Staff #2 (DOH 1/24/22) TB screening was conducted on 5/25/2021.
Evidence: 1. The Preschool 3 class (3 years) has an unlocked cabinet under the changing table with cleaning products and a bottle of bleach solution inside the cabinet.
Violation was corrected during the inspection.
Evidence: Staff #4 (Infant classroom) did not wash their hands before or after changing a child?s diaper.
Evidence: 1. Staff #4 (Infant classroom) did not use soap and water to clean the diapering surface after each use.
Evidence: The Toddler Staff did not sanitize the tables before children were fed their lunch.
