Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance.
On February 4, 2026, Child #1, 2, 3, and 4 (ages 6-10 years old) were unsupervised by center staff for approximately 12 minutes as they walked from the public-school bus stop, located approximately 50 yards away from the center. At the time of the report, sidewalks were snow/ice covered and not usable resulting in the children walking unsupervised in the street to get to the center. The posted speed limit is 25 mph. There is significant sun glare for drivers and busy traffic patterns are present in this area during the time this bus stop takes place.

On February 4, 2026, Staff #2 failed to be present at the offsite bus stop located approximately 50 yards from the center, to receive child #1, 2, 3, and 4, (ages 6-10 years old). The children exited the bus at the bus stop at approximately 3:45 pm, and based on attendance sheets, were not present and supervised at the center until 3:58 pm. Witness A & B stated that this has happened at least two times prior to this.
