Inspector Notes

An unannounced monitoring inspection was conducted on 08/31/2022. There were 9 children, ages 5 months-4 years, at a total of 30 points and 2 staff members present during the inspection. The inspector observed the following: free choice activities, diaper change, bottle feeding, and nap time. A total of 4 children?s and 2 staff records, were reviewed. The inspector arrived at 9:52 AM and departed at 11:15 AM.

Based on review of four children?s records, the licensee failed to ensure that all immunizations were updated every 6 months for children under the age of two.

Evidence:
Child # 1 most recent immunization record was 12/12/2021. The child?s DOB is 12/12/2021. There was no documentation of an updated immunization record or exemption
Plan of Correction: The licensee will speak with the guardian about obtaining an exemption. All records will be monitored.
