Inspector Notes

An unannounced on-site monitoring inspection was conducted on 03/06/2023. There were 10 children, ages 5 months-3 years at a total of 30 points, and 3 staff members present during the inspection. The inspector observed the following: free choice activities, table time, and napping. A total of 5 children?s records and 3 staff records, were reviewed. The inspector discussed the following with staff: hiring an additional staff person. The inspector arrived at 8:45 AM and departed at 10:40 AM.

Based on review of five children?s records, the center failed to ensure that all records were complete per the standard.

Evidence:

1.The record for Child # 3 was missing one of the two emergency contacts, and the parent?s work phone number.
2. The record for Child # 2 was missing documentation of allergies and action to take and previous schools attended.
Plan of Correction: Provider will have the parent to complete the missing information ASAP.
