Inspector Notes

An unannounced monitoring inspection was conducted on August 7, 2023 from approximately 11:15 am to 12:45 pm. There were 130 children in attendance and a total of 24 staff present. 10 staff records and 10 children?s records were viewed. All areas of the center were observed including classrooms bathrooms, and the playgrounds. The children were observed playing outside, eating lunch, swimming, and playing with table toys, while the licensing inspector was on-site.

The most recent Health Inspection was conducted on 02/16/23. The most recent fire inspection was conducted on 01/10/23. Evacuation drills were conducted monthly with the most recent on 07/28/23.

Violations were cited as a result of this inspection.

Please complete the plan of correction and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today, 08/24/2023. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

Based on record review, and interview, the center failed to ensure that each child shall have a physical examination by or under the direction of a physician within 30 days after the first day of attendance.
Evidence:
The record for child #3, date of enrollment 06/12/23, did not contain evidence of a physical examination. Interview with the Director confirmed that the child's record did not contain evidence of a physical.
Plan of Correction: We have reviewed expectations for child files. Parent was contacted and form received.
Standard 8VAC20-780-240-A
Based on records review, and interview, the center failed to ensure that the Virginia Department of Education sponsored orientation course shall be completed within 90 calendar days of employment.
Evidence:
The record for staff #1, date of employment 01/16/23, the record for staff #5, date of employment 04/11/2023, and the record for staff #6, date of employment 08/11/22, did not contain documentation of the Virginia Department of Education sponsored orientation course.
Plan of Correction: Staff have been instructed to complete these. Expectations have been reviewed with site Director.
