An unannounced, on-site monitoring inspection was initiated on 7/31/2024 and the inspection concluded on September 5, 2024. The on-site inspection began at 12:00 p.m. and ended at 2:30 p.m. The inspector reviewed compliance in the areas listed above. There were 169 children present and 17 staff. The inspector reviewed 10 children?s records on-site and 9 staff records electronically on September 5, 2024. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on September 25, 2024. A POC submitted after this date will not appear on the public website.

Orientation must be completed for each staff member no later than seven days of the date of assuming job responsibilities. Staff 1 did not have documentation of orientation on file until after 23 days of employment.

Staff shall complete annually a minimum of 16 hours of training appropriate to the age of children in care. There was no documentation to support that Staff 5 and Staff 8 had 16 hours of annual training in 2023/2024.

Each grouping of children shall have at least one staff who meets lead teacher qualifications. Evidence: Staff 1 is in the position of lead, but does not meet lead teacher qualifications. Staff 2 was alone with Huddle 3, but does not meet lead teacher qualifications. Staff 3 and Staff 9 were alone with Huddle 4; neither meet lead teacher qualifications. Staff 5 and Staff 7 were alone with Huddle 5; neither meet lead teacher qualifications.
