Inspector Notes

An unannounced inspection was initiated on 7/31/2024 and concluded on 9/04/2024 in response to a complaint received by the licensing office on 7/19/2024 relating to allegations of staffing and attendance records. This inspection included document review, a tour of the facility, and interviews.

The preponderance of evidence gathered during the investigation supports the allegation regarding attendance records; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 9/17/2024. A POC submitted after this date will not appear on the public website.

For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs. During the inspection on 7/31/2024, the attendance records were as follows:
In Huddle 6, there were 17 children physically present, but only 15 were marked present on the attendance record.
In Huddle 2, staff were not sure how many children were present; the inspector asked them to do face to name roll call. Thirty three children were physically present, but only 31 were marked present on the attendance record.
In Huddle 3, there were 16 children physically present, but 18 were marked present on the attendance record.
