Inspector Notes
Today an unannounced Monitoring Inspection took place between approximately 10:15 a.m. and 12:05 p.m. There were 7 classrooms observed with a total of 78 children with 13 staff within the supervision guidelines. The children were observed playing on the playground, playing with blocks, participating in a fitness class and other organized activities. A complete inspection of the physical plant, children and staff records, fire drill log, medication and emergency procedures were observed during this inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations
9Evidence:
1. Staff #1 (date of employment: 03/14/2022) had a TB screening dated 08/07/2020.
2. Staff #2 (date of employment: 05/09/2022) had a TB screening dated 10/14/2021.
Evidence:
1. In the Infant 2 classroom, one spray bottle of a bleach/water solution and one spray bottle of window cleaner were in an unlocked cabinet under the sink.
Evidence:
1. At the time of inspection, one outlet in the Toddler classroom did not have a protective covering.
2. At the time of inspection, one outlet in the Two?s classroom did not have a protective covering.
Evidence: Child #1 has a documented allergy and epi-pen. The posted allergy lists in each classroom did not list Child #1?s allergy.
Evidence:
1. In the Two?s classroom, there was one ointment that expired 12/2021.
2. In the Two?s classroom, there was one ointment that expired 02/2022.
Evidence: In the Three?s classroom, sunscreen belonging to Child #2 was not labeled with the child?s name.
Evidence: In the Infant 2 classroom, one diaper cream was not labeled with the child?s name.
Evidence: In the Three?s classroom, one insect repellant belonging to Child #2 was not labeled with the child?s name.
Evidence: In the Infant 1 classroom, two bottles belonging to Child #3 were not dated and labeled with the child?s name.
