Inspector Notes

Today an unannounced Monitoring Inspection took place between approximately 10:15 a.m. and 12:05 p.m. There were 7 classrooms observed with a total of 78 children with 13 staff within the supervision guidelines. The children were observed playing on the playground, playing with blocks, participating in a fitness class and other organized activities. A complete inspection of the physical plant, children and staff records, fire drill log, medication and emergency procedures were observed during this inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard 8VAC20-780-160-A
Based on record review, staff's documentation of a Tuberculosis (TB) screening was not completed within the last 30 calendar days of the date of employment.

Evidence:
1. Staff #1 (date of employment: 03/14/2022) had a TB screening dated 08/07/2020.
2. Staff #2 (date of employment: 05/09/2022) had a TB screening dated 10/14/2021.
Plan of Correction: Both staff have completed a new TB test.
Standard 8VAC20-780-280-B
Based on observation, hazardous substances such as cleaning materials were not kept in a locked place using a safe-locking method that prevents access by children.

Evidence:
1. In the Infant 2 classroom, one spray bottle of a bleach/water solution and one spray bottle of window cleaner were in an unlocked cabinet under the sink.

Standard 8VAC20-780-290-A-3
Based on observation, in areas used by children of preschool age or younger, not all electrical outlets had protective covers.

Evidence:
1. At the time of inspection, one outlet in the Toddler classroom did not have a protective covering.
2. At the time of inspection, one outlet in the Two?s classroom did not have a protective covering.

Standard 8VAC20-780-40-M
Based on observation, the center did not maintain, in a way that is accessible to a staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions.

Evidence: Child #1 has a documented allergy and epi-pen. The posted allergy lists in each classroom did not list Child #1?s allergy.
Plan of Correction: Child 1 name has been added to the allergy list.

Based on observation, over-the-counter skin products were kept beyond the expiration date of the product.

Evidence:
1. In the Two?s classroom, there was one ointment that expired 12/2021.
2. In the Two?s classroom, there was one ointment that expired 02/2022.
Plan of Correction: We will toss out the expired ointments.

Based on observation, not all sunscreen was labeled with the child?s name.

Evidence: In the Three?s classroom, sunscreen belonging to Child #2 was not labeled with the child?s name.

Evidence: In the Three?s classroom, one insect repellant belonging to Child #2 was not labeled with the child?s name.
