Inspector Notes
A monitoring inspection was initiated and concluded on April 19, 2022. There were 61 children present ages four months through five years old with 12 staff and one administrator. During the inspection a walking tour of the center inside and out was completed. A sampling of children and staff records were reviewed. Children were observed in teacher and self-directed activities including outside play, lunch, transitions, hand washing and rest period. Additionally, the fire, health inspection, emergency equipment and emergency evacuation drills were reviewed to determine if required documentation and equipment were in compliance with standards. Violations regarding physical plant emergency equipment, sanitation, crib labels, infant emergency supply and medication were observed and discussed with the program director during the exit interview at the conclusion of the inspection.

On the toddler playground there is a resilient surface depth of three inches where six inches are required for a climber that is 12 inches tall.
Evidence:
1 - In the three-year-old room a staff was heard using a raised voice to say SIT, SIT, SIT DOWN, PUZZEL, PUZZEL, as the staff stood over a child.
2 - In the toddler room a staff was heard saying in a raised voice "NO YOU DON'T, NO, NO STOP!
3 - In the four-year-old room a staff was heard saying in a raised voice "MY DIRECTIONS WERE TO SIT ON THE CARPET, SIT ON THE CARPET, SIT ON THE CARPET, NOT IN MY CHAIR!
Evidence:
Child #1 was resting a crib identified for Child #2.
Evidence:
1 - Staff #2 was observed working on top of the changing table with a tablet and clipboard resting on the table as she worked.
2 - The foot operated diaper disposal system is not operational and staff must use their hand to open the trash can to throw soiled diapers in.
Evidence:
1 - There was not a battery operated radio or flashlight available during the inspection.
2 - Staff #1 stated that there was not a battery operated radio or flashlight available during the inspection.
Evidence:
There were two disposable bottes and three formulas available for seven bottle fed infants during the inspection.
