An unannounced inspection was initiated on 12/18/2025 and completed on 2/12/2026 in response to a complaint received by the licensing office on 12/17/2026 relating to allegations of Staffing and Supervision and Programs. This inspection included document review, tour of the facility, interviews, and observations.
The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
