Inspector Notes
An unannounced renewal inspection was conducted on 9/12/2022 between 12:15pm and 2:00pm. There were 12 children present with the Provider and 2 Assistant for a total of 38 points within the supervision guidelines. The children were observed freely engaging and interacting. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children, household members, and staff records, evacuation drills, and attendance were observed during this inspection. Thank you for your cooperation. Violations
4Evidence:
1. The record for caregivers #2 and #3 did not contain documentation of the 16 hours of required annual training.
2. The Provider confirmed that Caregivers #2 and #3 did not complete the required 16 hours of annual training.
Evidence:
1. In a bathroom, a container of air freshener was located on top of the toilet accessible to children.
2. In the Infant area, a container of disinfectant spray was located on a low counter accessible to children.
Evidence:
1. The provider did not have documentation of a current annual fireplace inspection.
2. The most recent fireplace inspection is dated 6/25/2021.
Evidence: 5 of 5 topical skin products review did not have written authorizations for use.
