Inspector Notes

An unannounced monitoring inspection was conducted on 3/31/2023 from 9:55 am-11:08 am. There were 12 children (31 points) directly supervised by 3 caregivers. The home, 3 caregiver records, 5 children?s records, 2 household member records, evacuation drills, and policies were inspected. Areas of non-compliance are identified in the Violation Notice. Standard 8VAC20-800-280
Based on observation, potentially poisonous substances and materials were not stored away in an area inaccessible to children.

Evidence: A container of air freshener and disinfectant spray was found under a sink cabinet in the children's bathroom.
Plan of Correction: Removed during the inspection and I will put the lock back on.
Standard 8VAC20-800-760-A-1
Based on observation, the center did not ensure each first aid kit had all the required items.

Evidence: Two triangular bandages were missing from the first aid kit.
Plan of Correction: Not available online. Contact Inspector for more information.

Based on record review and interview, the provider did not annually review with the parent the emergency contact information to ensure the information is correct and obtain the parent's signed acknowledgment of the review.

Evidence:
1. Child #1's last annual emergency contact review was on 1/13/2022.
2. Child #3's last annual emergency contact review was on 10/30/2021
Plan of Correction: I will have parents initial and date.

Based on record review and interview, proof of identity was not obtained within 7 days.

Evidence:
1. The birth certificate number and issue date for child #1 are missing from the proof of identity documentation on file.
2. The date of enrollment for child #1 is 1/17/2022.
