Inspector Notes

An unannounced monitoring inspection was conducted on 3/26/2024 from 9:57 am to 10:53 am. There were 11 children supervised by 3 direct caregivers. The home, 3 caregivers records, 3 children?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed engaging freely. Areas of non-compliance are identified in the Violation Notice. Please submit your plan of correction within 5 business days for it to be included on the website. Thank you for your cooperation during this inspection. Based on record review and caregiver interview, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.

Evidence: There was no documentation of a physical examination completed by or under the direction of a physician prior to or within 30 days of Child #2's first day of attendance. The start date for Child #2 is 2/12/2024.
Plan of Correction: Received the completed physical examination for child #2 signed by the physician.

REPEAT VIOLATION

Based on observation and caregiver interview, potentially poisonous substances and materials were not stored away in an area inaccessible to children.

Evidence: A container of air freshener and nail hole patch tube was found under the sink cabinet in the children's bathroom.
Plan of Correction: Removed the air freshener and nail patch tube. I put a lock on the door.

Based on observation and caregiver interview, small electrical appliances were not unplugged when not being used.

Evidence: A toaster oven in the kitchen was plugged in and not being used.
Plan of Correction: Will make sure toaster oven is unplugged when it is not being used.
