Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 3/12/2025 and completed on 3/12/2025. The on-site inspection began at 9:57 am and ended at 10:50 am. The inspector reviewed compliance in the areas listed above. There were 12 children present and 3 caregivers; the point total was 38. The inspector reviewed 5 children?s records and 3 caregiver/household member(s) records on-site. This inspection included document review, a tour of the home, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 3/19/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-770-70-A
Before the child's first day of attendance, parents shall be provided required information in writing.

The liability insurance form was not signed by Child #1's parent. Child #1 had been in care for over two months.
Plan of Correction: Parent of child#1 signed the form.
Standard 8VAC20-790-180-A
A tuberculosis (TB) screening is required every two years from the last screening.

An updated TB screening for Caregiver #3 was not available at the time of the inspection. The TB screening was overdue by 6 days.
Plan of Correction: Caregiver #3 had an appointment on Feb 25th & March 4th at the FCHD. He got the flu and Norovirus. FCHD postponed his appointment to March 11th. They did a TB blood test and I received the negative results today.
