Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A subsidy health and safety inspection was initiated on April 16, 2021 and concluded on April 16, 2021. The provider was contacted by telephone to initiate the inspection. There one child present and one caregiver. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection.

The information gathered during the inspection determined non-compliance(s) with subsidy regulations, and violations were documented on the violation notice issued to the facility.

Based on review of caregiver records and interview, the provider did not ensure caregivers who work directly with children, in addition to preservice and orientation training required in subsections A through D of this section, annually attend at least 16 hours of training, to include the department's health and safety update course.

Evidence:

1. The training log for caregiver #1 indicated the health and safety update training was completed on 04/12/2019 and 04/14/2021. The provider stated the annual annual health and safety training was not completed for the 2020 year.
2. The training log for caregiver #2 indicated the annual health and safety update training was completed on 04/12/2019. The provider stated the annual health and safety training was not completed for the 2020 year.
3. The training log for caregiver #3 indicated the annual health and safety update training was completed on 05/06/2019. The provider stated the annual health and safety training was not completed for the 2020 year.
Plan of Correction: Per the provider: We have completed the annual training and will continue to do so each year.
