Inspector Notes

An unannounced monitoring inspection was conducted today, June 28, 2023. The inspector was on site from approximately 12:00pm until 1:30 pm. There were ten children present with two caregiver supervising for a value of 17 points. Children were observed playing with
balls, the kitchen set and blocks. Later children had prepared for nap.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of eight children?s records, two caregiver records and two household member records were
reviewed. The inspectors interacted with the caregiver and children in care.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-800-100-A
Based on review of eight children?s records, the provider did not obtain documentation of the
child?s physical prior to the child?s attendance or within 30 days after the first day of attendance for two children?s records.

Evidence:

1. The record of Child #3, enrolled 01/23/2023, did not have documentation of a physical
exceeding 30 days from the first day of attendance.
2. The record of Child #6, enrolled 01/03/2023, did not have documentation of a physical
exceeding 30 days from the first day of attendance.
Plan of Correction: Per the provider: I will ask the parent to bring me a copy of the physical.
Standard 8VAC20-800-240-A
Based on observation, the provider did not ensure areas and furnishing of the family day
home, inside and outside, were maintained in a clean, safe, and operable condition.

Evidence:

Two sheets of wooden latus were observed on the ground near the slide. The latus had multiple nails that were protruding upward and posed a risk of puncturing a child in care.
Plan of Correction: Per the provider: I will have the latus removed from the yard.

Based on review of eight children?s records, the provider did not annually obtain the parents signed acknowledgement of the emergency contact information review.

Evidence:

1. The most recent acknowledgement of the emergency contact information for child #4 was dated 03/18/2018, exceeding the annual requirements.
2. The most recent acknowledgement of the emergency contact information for child #8 was dated 04/03/2017, exceeding the annual requirements.
Plan of Correction: Per the provider: I will have the parents review the record and sign it.
