During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated on March 30, 2026 and completed on March 30, 2026. The on-site inspection began at 10:45 AM and ended at 12:15 PM. The inspector reviewed compliance in the areas listed above. There were five children present and one caregiver; the point total was 11. The inspector reviewed five children?s records and two caregiver records on-site. This inspection included document review, tour of the facility, interviews, and observations.
A subsidy health and safety inspection (SHSI) was conducted in conjunction with the licensing inspection. Subsidy standards that are not covered in licensing standards were reviewed during this SHSI.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on April 7, 2026. A POC submitted after this date will not appear on the public website.
Violations
51. The wooden jungle gym had splintered wood that was raised above the surface that could snag the skin of a child in care.
2. The caterpillar play tunnel and the plastic water table were broken in multiple areas.
3. The spring rocking horse stabilizing frame had one broken leg.
1. In the bathroom children use, one container of mouthwash was sitting on the sink counter and two cans of air freshener were stored in reach of children.
2. One container of antifreeze was being stored on the floor of the deck leading to the outdoor play area.

The provider did not have documentation of attendance for one of the five children.
Caregiver #1 removed a soiled diaper and did not wash their hands.
A soiled diaper was sitting on a baby Exersaucer activity table when the inspector arrived.
