Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was initiated on 02/18/2022 and concluded on 02/23/2022. The in-charge person was contacted by telephone to initiate the inspection. There were 69 children present and 10 staff. The inspector emailed the director/provider a list of items required to complete the inspection. The Inspector reviewed 4 children?s records and 4 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-160-A
Based on record review, the licensee did not ensure there was a documentation of negative tuberculosis screening.

Evidence : Staff #3 documentation provided for tuberculosis screening did not have the complete date when the screening was complete or clear results.
Plan of Correction: New leadership has been trained on all new hire requirements to meet this criterion. Employee will have a current TB on file by 3/1/22. DM will review all new hire paperwork for the next 45 days to ensure compliance

Based on record review, the licensee did not ensure the most recent inspection of the facility was posted. .

Evidence: The licensee most recent inspection was not posted for March 25, 2021 . The inspection posted was October 6, 2020.
Plan of Correction: 120-E-2: This was corrected the day of the visit. New leadership has been trained on all licensing/ DOE standards. DM will review for compliance at all school visits.
