Inspector Notes

An unannounced monitoring inspection was conducted on September 14, 2023 from 2:30 pm- 4:50 pm. At the time of entrance, 50 children were in care with 8 staff members present. Additional staff was present to assist. The sample size consisted of 5 children?s records and 5 staff
records. Children were observed transitioning to snack time and afternoon activities.

Areas of non-compliance are identified on the violation notice.

Standard 22.1-289.035-B-4
Based on record review and staff interview, the center did not ensure to obtain a copy of the results of a criminal history record information check, a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.

Evidence:
Staff #1 hire date 08/14/2023.
Staff #1 lived in Pennsylvania in the past 5 years.
Staff #1 did not have Pennsylvania criminal history record information check prior to the 1st day of employment.
Plan of Correction: Background Check has been received . Center Director will ensure that all required background checks are completed prior to employee's first day of work .
Standard 8VAC20-780-160-A
Based on record review and staff interview, the center did not ensure each staff member and shall submit documentation of a negative tuberculosis screening. Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department

Evidence : Staff #4 hire date 07/10/2023. Staff #4 most recent tuberculosis test was 04/28/2023 which is not with in the last 30 days of the date of employment.
Plan of Correction: Updated TB test been received . School Management will ensure that all required documents are received no later than the employee's first day of work.
Standard 8VAC20-780-270-A
Based on observation, the center did not ensure areas and equipment of the center, inside and outside shall be maintained in a clean, safe and operable condition.

Evidence:
The bathroom partition in the preschool is rusting.
Outside on the playground , there is a cone that is cracked .
Outside on the building, there is splintering siding.
Plan of Correction: Work Orders have been placed and repairs should be completed by 11/15/2023.
Safety Captain will complete daily playground checklists and report any areas of concern to school management.

Based on record review, the center did not ensure docuemntation shall be maintained of emergency evacuation, shelter in place and lock down drills .

Evidence:
There was not documentation of June emergency evacuation drill was conducted.
Plan of Correction: Drill log has been corrected . School Management will ensure that all drills are documented as they occur.

Based on record review and staff interview, the center did not ensure each child record contained all required information.

Evidence:
Child #2 and Child #5 record documentation did not have complete emergency addresses for each emergency contact.
Plan of Correction: Emergency contact information has been obtained. School management will conduct paperwork appointments prior to child's start date to ensure all required information has been collected.
