Inspector Notes
An unannounced inspection was initiated on Novemer 21, 2024 and completed on November 25, 2024, in response to a complaint received by the licensing office on November 12, 2024 relating to allegations of Staffing and Supervision, Administration and Programs. The Department issued a Special Order on May 3, 2024 to place licensee on probation for a period of nine months, and require the licensee to contact parents, guardians, or other responsible persons (familes) regarding health and safety violations as a result of high-risk violations. This inspection included document review, tour of the facility, interviews and observations.
The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
