Inspector Notes

An unannounced monitoring inspection was conducted today from 9:45am-10:30am. There were 24 children directly supervised by 4 staff. The physical plant, 5 staff records, 6 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in a safety presentation. There was an assortment of age appropriate materials for the children in care. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice.

REPEAT VIOLATION
The licensee is to request the results of a criminal record check from all non- NFF US states and territories that an employee has resided in, prior to the employee being offered employment. Staff 2 was observed working on the date of inspection and the staff member's file did not contain the results of a out of state criminal record check from a non-NFF state the employee has resided in within the past five years.
Plan of Correction: Employee has scheduled an out of state criminal record check. Waiting on results.

REPEAT VIOLATION
The licensee is to maintain children's records with all required information. Child 6's record did not contain the listed parents' place of employment or the names, addresses, and phone numbers of two emergency contacts.
Plan of Correction: Contacted parents to get necessary information on emergency form.
Standard 8VAC20-780-70
The licensee is required to maintain staff files with required information. Staff 1, 2, 3, 4, and 5's record did not contain documentation of date of hire, an emergency contact, two or more references, and resume or application, and verification of education and experience.
Plan of Correction: Staff files are on site and completed with correct documentation.
