Inspector Notes

An unannounced renewal inspection was conducted on February 6, 2023 between the hours of 9:15am and 11:50am. There were 32 children present, with 7 staff supervising within the supervision guidelines. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 4 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-770-60-C-2
Based on record review, the central registry finding was not received by the 30th day of employment.
Evidence: Staff #2 (DOH 1/20/22) did not have central registry results within 30 days of employment.
Plan of Correction: We will be sure to send the form immediately and follow up with the agency if not received within 30 days.
Standard 8VAC20-780-160-A
Based on record review, the center did not obtain documentation of tuberculosis screening of each staff member at the time of employment and prior to coming into contact with children.

Evidence: Staff #2 (DOH 1/20/22) does not have tuberculosis screening results on file.
Plan of Correction: I will speak with the teacher and I will send her home until we receive the TB screening results.
Standard 8VAC20-780-430-H
Based on observation, the center failed to ensure trampolines were not used by children in the program.

Evidence: The license inspector observed a child jumping on a small, exercise trampoline located in the PreK classroom.
Plan of Correction: We will remove the trampoline today.
Standard 8VAC20-780-80-A
Based on record review and interview, the center did not maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.

Evidence:
1. The PreK 2 (age 2) did not have a written record of daily attendance.
2. The Director confirmed teachers do not document the arrival and departure of each child as it occurs. The teachers receive children between 7:30-820am on the playground and teachers will check the attendance upon arrival in the classroom at that time.
