The child day center shall obtain subsequent background checks for each employee every five years.

The most recent central registry background check for staff #1 is dated 3/25/2020. The background check has been expired for approximately 9 months.

Staff members shall submit subsequent tuberculosis (TB) screening results every two years from the date of the initial screening.

Staff #3s most recent TB screening is dated 7/21/2023. The TB screening has been expired for approximately 4.5 months.

Staff #1 and staff #2 have not completed orientation training. The staff have been employed for approximately 5 years and 2 years, respectively.

When an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days will be disposed.

An authorization for child #5s medication was signed by the childs parent 8/25/2025 and the authorization expired on 9/8/2025. The medication was observed on-site.
