Inspector Notes
A self report by facility administration was received on 4/18/23. An inspection was conducted on 4/21/23. Areas reviewed are listed above. There were 2 violations as a result of the self reported incident.

Based on review of video the facility failed to ensure the care, protection, and guidance of the children as required.
Evidence:
On 4/14/23 at approximately 11:25am staff #1 failed to ensure the care, protection, and guidance of child #1 when she observed her standing on the lunch table. The staff did not speak to or go to the child to get her down, she watched her continue to stand on the table and then jump off directly towards another adjacent table where she landed feet first on the floor. This action posed a risk to the child, as the child could have hit her face on the adjacent table.
Evidence:
On 4/14/23 at approximately 11:25am staff #1 failed to ensure the care, protection, and guidance of child #1 when she observed her standing on the lunch table. The staff did not speak to or go to the child to get her down, she watched her continue to stand on the table and then jump off directly towards another adjacent table where she landed feet first on the floor. Plan of Correction: Upon notification of issues the administrative staff immediately reviewed the video footage and saw the unacceptable actions of the staff. The staff was removed from the site and brought into the office. Staff #1 is no longer employed with the agency as of 4/21/23.

Based on review of video the facility failed to ensure that rough handling of the children did not occur.
Evidence:
Video showed on 4/14/23 that staff #1 rough handled child #1 on 4 occasions:
1. Video showed at 11:26am staff #1 grabbing child #1 forcefully by the arm to remove her from the sink area.
2. At 11:29am staff #1 pushed child #1 away from the door 2 times while trying to keep the child from exiting with the other children.
3. At 11:30amstaff #1 then grabbed child #1 by the arm as the child was on the floor near the door and placed her foot under the child's bottom and lifted her up by the arm and her foothold and moved her to an area away from the door.
