Inspector Notes
An unannounced monitoring inspection was initiated and completed on 10/23/2024 in response to a self-report received by the licensing office on 10/18/2024 relating to Programs. The inspector reviewed one child record and one staff record on-site. This inspection included document review, interview and observation.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

The center failed to ensure that roughly handling of a child does not occur.
A review of video footage from 10/17/2024 shows Staff #1 picking up Child #1 under their arms, walking them over to their sleeping cot and dropping them onto the cot. Staff #1 leaves the room to retrieve another cot. When the staff member returns, they grab Child #2 by the right arm and shoves Child #2 onto their sleeping cot.

Plan of Correction: Administration reviewed the video footage resulting in dismissal from employment. Procedures and policies were followed during the internal investigation and proper agencies were notified of this incident through self reporting. Staff will be reminded and retrained if needed on appropriate actions and guidance of children.
