Inspector Notes
A renewal inspection was conducted on April 17, 2023. The inspectors arrived at 1:40pm and departed at approximately 3pm. Present in the home were 8 children, totaling 22 points, and two providers. The youngest child in care was 1 years old. Children were observed bottle feeding, naptime, diapering, and being picked up by their parent. First Aid kits and emergency preparedness documents were observed. A sample size of five children's records were reviewed, two staff records and household members. It was reported that no medication has been administered to children in care and no children's injuries were reported. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented. The violations are listed on the violation notice issued to the center and were reviewed with the two providers at the exit interview.

2. Staff #1 and Staff #2 confirmed that Household member #1 did not have the required criminal history record check.
Evidence:
1. Child #1 (date of enrollment 6/14/22) did not have documentation of a physical in their record.
2. Staff #1 and Staff #2 confirmed that Child #1 did not have documentation of a physical examination in their record.
Evidence:
1. During naptime, all 5 cots did not have the linens consisting of a top cover and a bottom cover.
2. Staff #1 and Staff #2 confirmed that the children were not using linens consisting of a top cover and a bottom cover.
Evidence:
1. Child #2, age 13 months, had a filled comforter blanket in their crib.
2. Staff #1 and Staff #2 confirmed that the child uses a filled comforter blanket.
