Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A subsidy health and safety inspection was initiated on March 31, 2021 and concluded on March 31, 2021. The director was contacted by telephone to initiate the inspection. There were 11 children present and eight staff. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection.

The information gathered during the inspection determined non-compliance with subsidy regulations, and a violation was documented on the violation notice issued to the facility.

Based on review of three staff records and interview, the vendor did not ensure three staff completed the department's health and safety update course.

Evidence:
The records for Staff #1 (hire date 04/23/2018), Staff #2 (hire date 07/29/2019) and Staff #3 (hire date 10/29/2019) did not contain the Virginia Annual Health and Safety Update Training. Subsidy Vendors and staff were required to have a certificate of completion for this training on file by March 1, 2020. Staff #4 stated the caregivers have not completed the Virginia Annual Health and Safety Update Training as of 03/31/2021.
Plan of Correction: I will have the staff complete the required training. We just reopened for in person learning in March 2021.
