Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A subsidy health and safety inspection was initiated on October 7, 2021 and concluded on October 15, 2021. The director was contacted by telephone to initiate the inspection. There were three children present and three staff. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection.

The information gathered during the inspection determined non-compliances with subsidy regulations, and violations were documented on the violation notice issued to the facility.

Standard 22VAC40-665-540-A
Based on review of three staff records and interview, the vendor did not ensure two staff members obtained a tuberculosis (TB) screening that had been completed within the last 30 calendar days of the date of employment.

Evidence:
1. The record for Staff #1 (hire date 03/08/2021) contained a TB screening dated 03/12/2021.
2. The record for Staff #2 (hire date 06/21/2021) contained a TB screening date 06/29/2021.
3. The director confirmed this information was accurate.
Plan of Correction: I had the conservation with our HR person in regard to TB screenings now being required prior to an employee being hired and not being older than 30 days.
Standard 22VAC40-665-580-H
Based on review of three staff records and interview, the vendor did not ensure one staff completed the department's health and safety update course.
The record for Staff #3 (hire date 07/01/2019) did not contain documentation that the Virginia Annual Health and Safety Update Training was completed. A member of management confirmed Staff #3 had not taken the required training as of 10/15/2021.
Plan of Correction: Staff #3 will complete the required training.
