Inspector Notes
The inspector conducted an unannounced monitoring inspection on April 13, 2023 to determine the center?s compliance with licensing standards. The inspection began at approximately 10:00AM and concluded at 12:10PM. There were 25 children present, with 9 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Violations
3Evidence:
1. In the 2?s and 3?s classroom, there was a hole in the wall approximately 2-3 inches located on the back wall under the window seal.
Evidence:
1. The Preschool classroom (Ages 3years ? 5 years) had one outlet that was not covered with an outlet cover located on the wall near the door within reach of children.
2. In the entry area above a bench, an outlet was not covered with and outlet cover. The outlet cover is within reach of children.
Evidence:
1. The allergy list was not kept in a confidential manner, as the list was observed posted on the front wall near the door in the 2?s and 3?s classroom.
