Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A subsidy health and safety (SHSI) inspection was initiated and concluded on 8/26/2021. There were 35 children present with 17 staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of three staff records, the center did not obtain documentation that a tuberculosis test of screening had been completed within the last 30 calendar days of the date of employment.
Evidence:
1. Staff #2 (hire date 2/01/2021) had a tuberculosis test or screening dated 10/10/2019.
2. Staff #3 (hire date 6/21/2021) had a tuberculosis test or screening dated 10/02/2020.
Plan of Correction: I will ask them to get an updated tuberculosis test or screening.
