Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program .

A renewal inspection was initiated and concluded on 8/26/2021. The director was contacted by telephone and a virtual inspection was conducted. There were 35 children present, ranging in ages from 5 months to 5 years old, with 17 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 3 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of three children's records, the center did not have immunization documentation signed or stamped by a physician, his designee, or an official of a local health department.
Evidence:
1. Child #2 had an immunization record, but it was not signed or stamped by a physician, his designee, or an official of a local health department and it was not on the form required by the Virginia Department of Health or a physician's form .
Plan of Correction: I will contact the parent and request it.

Based on record review and interview, the center did not maintain a record of the dates of all practice drills for one year.
Evidence:
1. The center listed one shelter in place drill for 2020, but Staff A stated they completed two shelter in place drills in 2020.
Plan of Correction: We will make sure we complete two shelter in place drills each year and document them.
