Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A renewal inspection was initiated on September 3, 2021 and concluded on September 7, 2021. The director/provider was contacted by telephone and a virtual inspection was conducted. There were 24 children present, ranging in ages from 2 years 11 months to six years of age, with four staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and seven staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 22VAC40-185-130-A
Based on a review of children's records, the facility failed to obtain documentation of immunizations prior to the first date of attendance.

Evidence:
1. Child #1's file did not contain documentation of immunizations. Date of enrollment for Child #1 was 08/27/2021.
2. Administration verified that there was no documentation of immunizations on file and the date of enrollment for Child #1.
Plan of Correction: Administration will contact the parents and if immunizations are not received the child will be unenrolled effective 09/10/2021. In the future children will not be able to attend until proof of immunizations are provided.
Standard 22VAC40-185-60-A
Based on a review of children's records, the center failed to obtain all required information for children records.

Evidence:
1. Child #1, Child #3 and Child #4's file did not state who has legal custody of the child.
2. Administration verified that the registration forms for Child #1, Child #3 and Child #4 did not contain information as to who has legal custody of the child.
Plan of Correction: Administration will communicate with parents to obtain the required information. In the future all forms will be checked by two different staff to ensure completion of all information.
Standard 63.2(17)-1720.1-B-2
Based on review of staff files, the center failed to ensure that a fingerprint based national background check was completed for staff prior to employment.

Evidence:
1. Staff #1's fingerprints were dated 08/20/2021. First day of employment for Staff #1 was 08/16/2021. Date of orientation for Staff #1 was 08/13/2021.
2. Administration verified the date of the fingerprints and first day of employment for Staff #1.
Plan of Correction: Staff were given the instructions to complete fingerprints and failed to do so in a timely manner. In the future administration will ensure that all staff have all required background checks needed prior to employment.
