During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated on 8/11/2025 and completed on 8/15/2025. The on-site inspection began at 9:30am and ended at 12:05pm. The inspector reviewed compliance in the areas listed above. There were 272 children present and 21 staff. The inspector reviewed 10 children?s records on-site and 10 staff records electronically on 8/14/2025 and 8/15/2025. This inspection included document review, tour of facility, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 8/26/2025. A POC submitted after this date will not appear on the public website.
Violations
6The center is required to obtain background checks from any state in which the individual has resided in the preceding five years.
Staff #10, employed for over 2 months, did not have documentation of requesting a central registry check or national sex offender registry check from one state. Staff #9, employed for over two months, did not have documentation of requesting a central registry check or national sex offender registry check from one state.
Staff #4, employed for over 1 year, has not completed the online orientation course.
Staff #5, #6, #7, and #8, who did not meet lead teacher qualifications, were alone with each of their groups of school age children for the duration of the inspection.
The medication authorization for a child shall be available to staff during the entire time it is effective.
The record for child #2 did not contain a complete medication authorization for one prescription medication stored for the child on-site.
Staff #2 was alone with a group of eleven, school age children and was not certified in CPR or first aid.
Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information.
The record for child #1, enrolled for one day, was missing the following required components: the work phone number and place of employment of each parent. The record for child #3, enrolled for 7 weeks, was missing the following required components: the work phone number and place of employment of each parent. The record for child #4, enrolled for 5 weeks, was missing the following required components: the work phone number and place of employment of each parent. The record for child #7, enrolled for one day, was missing the following required components: the name, address, and phone number of one designated person to call in an emergency. The record for child #8, enrolled for one day, was missing the following required components: the name, address, and phone number of one designated person to call in an emergency. The record for child #10, enrolled for 6 weeks, was missing the following required components: the work phone number and place of employment of each parent.
