Inspector Notes
An unannounced monitoring inspection conducted on August 24, 2023 between the hours of 11:30am and 2:00pm. There was a total of 37 children with 7 staff members supervising. Children were observed engaged in group activities, playing with various age-appropriate toys, riding trikes, playground activities, painting, reading, eating lunch, handwashing, and nap-time. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, children and staff records, special care and emergencies and nutrition. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

5Evidence:
1. The record for Staff #4 (DOH 6/28/23) did not contain a Virginia Central Registry finding.
2. The director confirmed that Staff #4 did not have a Central Registry finding in their record.
Evidence:
1. The record for Child #3 (date of enrollment 06/26/23) did not have documentation of a physical examination by a physician within 30 days after the first day of attendance.
Evidence:
Child #4 had an emergency allergy medication on site but did not have the physician's authorization as required for long-term prescriptions.
Evidence: The medication authorization for child #5 was expired (expired November 2022), but the medication was still at the center.
Evidence: The records for Staff #4 (DOH 6/28/23) did not have documentation of two (2) or more references before employment.
2. The Director acknowledged there was only one (1) reference check for Staff #4.
