Inspector Notes

An unannounced Monitoring Inspection took place on January 22, 2024, between the hours of 10:00am and 11:50am. There were four (4) classrooms observed with a total of 38 children present with 8 staff supervising within the supervision guidelines. The children were observed playing with various manipulatives, painting, music, toileting, handwashing and eating lunch. A complete inspection of the physical plant, children and staff records, evacuation logs, medication, required posted documents, first aid kit, and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Based on observation, the staff did not wash their hands with soap and running water before and after helping a child use the toilet.
Evidence: Staff # 5 did not wash their hands before and after helping children use the toilet.
Plan of Correction: I will retrain my staff again on proper handwashing procedures.
Standard 8VAC20-780-550-E
Based on record review, shelter in place procedures were not practiced a minimum of twice a year.
Evidence: There was one documentation of a shelter-in-place procedure on 9/29/23.
Plan of Correction: I will ensure we practice shelter in place procedures twice a year.
Standard 8VAC20-780-550-F
Based on record review and interview, lockdown procedures were not practiced at least annually.
Evidence: 1. No documentation for a lockdown procedure practiced.
2. The center director acknowledged there was not a separate lockdown procedure practiced.
Plan of Correction: I will ensure we practice lockdown procedures annually.
