Inspector Notes

An unannounced complaint investigation was conducted on 8/5/2022 regarding special care provision and emergency , programs, administrative and staffing and supervision. Interviews were conducted and and observations were conducted. The preponderance of evidence support the allegations in regards to
special care provision and emergencies and the complaint has been deemed valid.

Based on staff interview, the licensee did not ensure the center notify the parent by the end
of the day of any known minor injuries.
Evidence:

Child #1 was injured on August 1, 2022 recieved a bandaid on elbow. Staff #2 confirmed he did not notify parent of injury and or the staff in charge.
Plan of Correction: Staff retraining on injury reporting.

Based on record review and staff interview, the licensee did not ensure the center maintain a
written record of children's serious and minor injuries in which entries are made the day of
occurrence.
Evidence:
Staff #1 and Staff #2 confirmed there was not an injury report for child #1 completed on the day
of occurrence.
Plan of Correction: Staff retraining on injury reporting.
