Inspector Notes

An Renewal unannounced monitoring inspection was conducted on November 3, 2023 from 11:00am - 2:00 pm. At the time of entrance, 25 children were in care with 4 staff members present. Additional staff was present to assist. The sample size consisted of 5 children?s records and 5 staff records. Children were observed preparing for naptime and taking bathroom breaks. No medication and transportation at the center, Outside area was inspected . The school age and preaschool areas were inspected.

Based on observation, the licensee did not ensure all areas and equipment inside and outside be maintained in clean and safe and operable condition.

Outside next to the preschool playground, the wooden walkway there was 8 boards warped they could cause a tripping hazard.
