Inspector Notes
An unannounced renewal inspection was conducted today between the hours of 8:30am-1:30pm. There were a total of 82 children with 12 supervising staff members present. Children were observed during breakfast, handwashing, centers, engaged in group activities and outdoor play time. A selection of staff and children records, medications and other documents were reviewed. The physical space, emergency supplies, evacuation drills, attendance records, and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Violations
7Evidence:
1. Staff #5's record (Date of hire-05/04/23) had documentation of a negative tuberculosis screening dated 01/16/23.
Evidence:
1. In the Toddler Room, there is peeling and chipped paint along the wall in the sensory area and the carpet is unraveling along the edges.
2. In the School Age classroom, the seam of the carpet has unraveled in the library area.
3. In the Preschool classroom, the carpet is unraveling along the edges in the home living area.
4. In the Early Preschool classroom, the rug located in the block area has unraveled exposing the fibers of the carpet.
5. In the Two's classroom, there is chipped paint and an inch and a half hole located in the wall near the block area of the classroom.
Evidence:
1. In the Infant classroom, a purse was on the ground under the high chairs that contained deodorant and cosmetics.
Evidence:
1. The mulch located at the bottom of the fall zones on the playground was not maintained in such a manner that it complies with the minimum safety standards of the depth of six inches.
Evidence:
1. In the two year old classroom, there was total of 12 children with two supervising staff. The youngest child in the classroom was under the age of two years old; therefore lowering the ratio to 1:5.
Evidence:
1. The Early Preschool classroom has a mixed age group of 2 1/2 and 3 year old children. The Early Preschool classroom contained a total of nine children with one supervising staff member. The ratio for the classroom is 1:8.The youngest child in the classroom was 2 1/2 years old; therefore exceeding the 1:8 ratio requirement.
2. Staff #6 confirmed that the Early Preschool classroom maintains a 1:8 ratio.
Evidence:
1. In the infant classroom, Child #6(14 months) hands were not washed before eating breakfast.
