Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on January 28, 2025, as a part of the licensure period. The on-site inspection began at 9:00am and ended at 11:15am. The inspector reviewed compliance in the areas listed above. There were 68 children present and 15 staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-40-E
The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with these standards, the center's own policies and procedures that are required by these standards.

Diapering procedures require that staff "put clean vinyl gloves on both hands and put clean diaper on child". Staff #6 did not put on clean vinyl gloves after removing soiled diaper.
Plan of Correction: Staff will be retrained on diapering procedures.

Repeat Violation

Children's hands shall be washed with soap and running water after toileting and any contact with blood, feces, or urine.

A child's hands were not washed with soap and running water after a diaper change.
