Inspector Notes

An unannounced subsidy inspection was conducted on 4/5/23. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection. Areas of noncompliance are noted in the violation narrative and were discussed with the provider during the exit meeting at the conclusion of the inspection.

Based on record review and interview it was determined the provider did not ensure they maintained a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs.
Evidence:
1. The sign in sheet did not contain the names and times of arrival for 5 of the 6 children present during the inspection.
2. Provider confirmed that they had not documented the arrival of 5 of the 6 children in care for that day.
Plan of Correction: Provider will document each child's arrival and departure as it occurs.
