Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 1/7/2025. The on-site inspection began at 10:25am and ended at 11:45am. The inspector reviewed compliance in the areas listed above to include standards found out of compliance during the previous inspection. There were 2 children present and 1 caregiver; the point total was 6. The inspector reviewed 3 children?s records and 2 caregiver/household records on-site. This inspection included document review, tour of the facility, interviews, observations and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Children's records are required to contain parental home and employment addresses and phone numbers. The record for child #1 was missing both parents work phone numbers. The record for Child #2 was missing the parent employment address and phone number. Both children were present during the inspection.
Plan of Correction: The provider will contact the parents to receive the missing employment information.
