Inspector Notes

An unannounced monitoring inspection was completed from 12:15 until 1:00 at the site. There were 7 children and two staff present.

The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of four children?s records were reviewed virtually.

Information gathered during the inspection determined non-compliance with applicable standards or law and violation were documented on the violation notice issued to the program.

Based on observation and measurement, the required depth of resilient surfacing was not maintained.
Evidence:
1. Approximately 5 feet In front of the swings and in back of the swings, the pea gravel measured less than the 6 inches required. Bare dirt was showing in some spots.
