Inspector Notes

An unannounced renewal inspection was initiated and completed on 4/16/2024 from 10:30am until 11:40pm. There were 14 children and three staff present. Five children?s records and three staff records were reviewed. Children were observed finishing lunch, walking back to the classroom, brushing teeth and washing hands, and playing.

The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition.

Information gathered during the inspection determined non-compliance with applicable standards or law and violation (s) were documented on the violation notice issued to the program.

This is a repeat violation.
Based on observation and measurement, the resilient surfacing did not meet the resilient surfacing measurement.
Evidence:
In the front and back use zone of the swings pea gravel measured approximately 0-4 inches. The use zone of swings is twice the height of the swing beam in front and back of the swing. The required depth is 6 inches.
Plan of Correction: Pea gravel will be distributed at the required depth.
