Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on May 18, 2021 and concluded on May 20, 2021. A self-reported incident was received by the department regarding allegations in the areas of staffing and supervision. The Director was contacted by telephone to conduct the investigation. The inspector emailed the provider a list of documentation required to complete the investigation.
The evidence gathered during the investigation supported the allegation of non-compliance with standards or law and violations were issued.

Violations
3Evidence:
1. Child #1 was left alone in a classroom for approximately 10 to 15 minutes.
2. Staff #1 and Staff #2 did not know that the child was left alone in the classroom. Administrator #2 received a phone call from a parent who was watching the classroom camera and reported that child #1 was alone in the classroom.
Evidence:
1. On 05/17/2021, the toddler class transitioned from their classroom to the playground. Staff #1 and staff #2 took the children to the playground. Child #1 was left alone in the toddler classroom during this transition.
Evidence:
1. Administrator #1 reported that ?Staff did not follow the written policy of transitions by the company and left a 17 month old child alone in a classroom?.
2. The center?s employee handbook was not followed when staff #1 and staff #2 left a child alone in the classroom. The center?s employee handbook states ?Appropriate employee conduct includes?, creating a warm, nurturing, and safe environment for the children in care?.
