Inspector Notes

Licensing Inspector conducted an unannounced Religiously Exempt Code Annual Inspection on October 24, 2023. There were 91 children ages 14 months to 5 years old present under the direct care of 12 staff. The Licensing Inspector reviewed 5 children?s records and 6 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.

Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, the center failed to ensure required Background Checks are completed every 5 years for every staff person.

EVIDENCE:
Staff #1 did not have an updated Central Registry Background Check within five years from the most recent Background Check completed on 9/01/2015.

Staff #3 did not have an updated Central Registry Background Check within five years from the most recent Background Check completed on 1/26/2018.

Staff #5 did not have an updated Central Registry Background Check within five years from the most recent Background Check completed on 4/26/2017.
Plan of Correction: Center Director will submit documents to update Background. Documentation of the findings will be maintained in the staff records.

Based on record review, the Center failed to obtain required out-of-state Background Checks for staff who are lived out of state within the past five years from the date of hire.

EVIDENCE:
Staff #2 lived outside of the state (New York) within 5 years before date of hire (2/27/2019), record did not include out-of-state Background Checks.

Staff #5 lived outside of the state (North Carolina) within 5 years before date of hire (1/30/2022), record did not include out-of-state Background Checks.
Plan of Correction: Out-of-state background checks will be submitted for each staff person who has lived outside of the state. documentation of the findings will be maintained in the staff record.
