Inspector Notes

A monitoring inspection was initiated and concluded on 4/17/2024. There were 4 children present with the provider and 1 household member supervising. The point total was 13. The children were observed during lunch time and rest time. A total of 4 child records, 1 staff record and 1 household member record were reviewed. The inspection started at 11:55 am and concluded at 1:05 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-800-460-D
Based on record review and provider interview the licensee did not ensure to maintain documentation of current rabies vaccination.

Evidence: The provider confirmed that one of their animals rabies vaccination was out of date by 3 months.
Plan of Correction: Provider is scheduling to take the animal to the vet.
Standard 8VAC20-800-530-B
Based on observation and provider interview the licensee did not ensure that when a crib was being used by infants it shall have a tight-fitting bottom cover.

Evidence: The provider confirmed that the bottom cover in the crib where child #1 (date of birth 9/5/2023) was placed for their rest period was loose, coming up around the edges, and was not tight-fitting.
Plan of Correction: The provider changed the sheet during the inspection.
