Inspector Notes
A renewal inspection was conducted on January 11, 2023 and concluded on January 12, 2023. The center was transitioning to after-school care. There were 10 children and 2 staff present. A walk-through of the physical plant was conducted of two classrooms.
Limited monitoring of child care requirements occurred during today?s visit. Licensing inspector monitored supervision of children, physical plant, staff/child ratios, adequate and approved space, programming, and nutrition. A review on medications on-site. A review of 2 child records, 2 staff records and 3 board officers records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations
5Evidence:
1. Agent #1 lived in District of Columbia (DC) within the last 5 years. The results of a criminal history check and sex offender registry check were not obtained from DC.
Evidence:
1. Staff #2 (date of employment 9/12/2022) did not have a completed sworn statement in the record available for review. Per staff interview, staff has worked at the facility since date of employment.
1. Staff #2 (date of employment 9/12/2022) did not have documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with children.

1. Staff #2 (date of employment 9/12/2022) and Staff #3 (date of employment 9/01/2015) did not have completed first day training recognizing child abuse and neglect and the legal requirements for reporting suspected child abuse.
Evidence:
1. Staff #2 and Staff #3 did not have documentation of two or more reference checks in record available for review during the inspection.
