During the inspection, the inspector reviewed the areas listed above, to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on March 20, 2025. The on-site inspection began at 9am and ended at 12:30pm. The inspector reviewed compliance in the areas listed above. The inspector reviewed 12 children?s records and 10 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.031-A
A child day center operated under the auspices of a religious institution, shall posted the fact that it is exempt from licensure in a visible location on the premises. The three separate buildings of the child day center did not have a posting that their center is exempt from licensure.
Plan of Correction: The center responded with the following: The center will post the Exemption Letter from the Department of Education on the parent board of all three separate buildings.
Standard 22.1-289.031-B-4
The center shall establish and implement procedures for ensuring that all children in the center are in compliance with the provisions of ? 32.1-46 regarding the immunization of children against certain diseases. Child #2 did have documentation or confirmation of being immunized against certain diseases.
Plan of Correction: The center responded with the following: During the time of the inspection, the director contacted the parent and they will provide a copy of the child?s immunization record to the school.
Standard 22.1-289.035-A
All employees shall undergo a background check and every five years thereafter. Of the 10 staff records reviewed, 9 of the 10 staff members has expired background checks.
Plan of Correction: The center responded with the following: Both the Director and Human Resources will ensure that all staff members have up to date background checks. Moving forward, all employees will have their background checks completed every five years.
Standard 22.1-289.035-B-4
Any individual required to undergo a background check shall authorize the child day center to obtain a copy of the results of a criminal history record information check, a sex offender registry check, and a search of the child abuse and neglect registry in which the individual has resided in the preceding five years. Staff #7 disclosed on her sworn statement that she lived in another state outside of Virginia the past 5 years and no out-of-state background checks were completed. Staff #7 was present at the center supervising children.
Plan of Correction: The center responded with the following: The center will obtain the out-of-state background checks of Staff #7 and place the results in her record as well as send a copy to the Department of Education.
Standard 22.1-289.049-A
The child day program shall obtain the proof of the child's identity and age within 7 business days of attendance. Child # 2 did not have proof of identity and age and has been enrolled over 7 business days.
Plan of Correction: The center responded with the following: The center called the parent during the time of the inspection, and they will bring the child?s birth certificate for them to obtain a copy for the child?s record.
